Here's How To Receive Your Ciba Vision Contact Lens Rebate:
1. Get a contact lens fitting and/or eye exam.
2. Purchase required number of boxes as indicated above.
3. Save your boxes and all receipts for rebate submission.
4. Go to cibavision-rebates.com and enter your individual rebate
code supplied by Daily-Contact-Lenses.com after your purchase.
5. Download and complete the Ciba Vision Official Rebate Form.
6. If you don't have access to a computer, call 877-886-4321 to
obtain your rebate. This will add two weeks to your processing time.
7. Mail the completed Official Rebate Form and all required proof of

Terms and Conditions
Maximum savings value up to $100 when combining the $15 rebate with
its respective product quantity purchase rebate. For example, get an
eye exam and/or fitting and (24) 30-pack boxes of Ciba Vision
Dailies Aqua Comfort Plus contact lenses and save $15 + $85 = $100 total. Valid on
purchases made between July 1, 2011 and December 31, 2012. Rebate
submission must be received by February 29, 2012. Product purchases
must be made within 90 days of exam. While supplies last. Rebate is
in the form of a Visa prepaid card, which expires 1 year after
issuance. Cards are issued by Citibank, N.A. pursuant to a license
from Visa U.S.A. Inc. and managed by Citi Prepaid Services. Cards
will not have cash access and can be used everywhere Visa debit
cards are accepted. CANNOT BE COMBINED WITH ANY OTHER REBATE OFFER.
Offer is limited to one rebate per person. Offer is not valid in
